We are seeking a
Senior Provider Success Manager
to serve as a strategic advisor to primary care organizations in New Jersey using Counterpart Assistant. This individual will own the success of a complex portfolio of provider partnerships and lead the strategies that drive sustained adoption, measurable performance improvement, expansion, and long-term retention.
This is a senior, results-oriented, provider-facing role for someone who can influence clinical and operational leaders, guide practices through meaningful workflow transformation, and connect product utilization to value-based care outcomes. The Senior Provider Success Manager will operate as an external product and industry expert while also representing the provider voice across Counterpart Health.
Beyond managing individual partnerships, this person will identify patterns across the market, develop scalable provider-success strategies, and mentor colleagues on effective engagement and change-management practices. The role requires strong judgment, comfort navigating ambiguity, and the ability to lead complex initiatives without direct authority.
Candidates must reside currently reside in
New Jersey
and be willing to travel to provider practices up to 85% of the time.
What You’ll Do
Own the strategy and performance of a complex provider portfolio
Develop and execute success strategies for a portfolio of high-priority or operationally complex primary care partnerships.
Establish shared clinical, operational, and business objectives with provider and practice leaders.
Maintain a comprehensive view of portfolio performance, including adoption, engagement, provider satisfaction, clinical opportunities, retention risks, and expansion potential.
Anticipate barriers and intervene early to protect partner value and long-term account health.
Lead business and performance reviews that translate data into clear priorities, decisions, and commitments.
Advise clinical and operational leaders
Build trusted relationships with physicians, advanced practice clinicians, practice administrators, and executive stakeholders.
Serve as a strategic advisor on incorporating Counterpart Assistant into clinical decision-making and value-based care operations.
Tailor recommendations to each organization’s workflows, patient population, staffing model, technology environment, and performance goals.
Influence stakeholders with different priorities and levels of technical or clinical expertise.
Lead sensitive conversations involving performance gaps, resistance to change, competing priorities, and partnership risk.
Lead adoption and clinical workflow transformation
Diagnose complex workflow challenges and design practical solutions that embed Counterpart Assistant into daily clinical operations.
Guide practices through implementation, change management, training, workflow redesign, and ongoing optimization on behalf of multiple customers.
Establish adoption plans with clear milestones, ownership, success measures, and escalation paths.
Identify opportunities to improve clinical and administrative processes across diverse primary care environments.
Ensure that adoption translates into sustained behavior change and measurable provider and patient value.
Drive measurable provider performance
Analyze utilization, operational, clinical, and financial data to identify trends, risks, and performance opportunities.
Delivery actionable scorecards and recommendations that help providers improve value-based care outcomes.
Lead targeted interventions for underperforming practices and evaluate their impact.
Shape regional multi-customer provider-success strategy
Identify recurring provider needs and convert successful approaches into scalable playbooks, tools, and engagement models.
Lead cross-portfolio initiatives that improve implementation quality, adoption, retention, or provider experience.
Establish and promote provider-success best practices across markets and practice types.
Use market insights and provider feedback to recommend improvements to Counterpart Health’s product, operating model, and go-to-market strategy.
Represent Provider Success in strategic cross-functional initiatives and planning discussions.
Partner across Counterpart Health
Collaborate with Growth, Implementation, Clinical, Product, Analytics, and Operations teams to deliver a coordinated provider experience.
Support high-value provider recruitment and expansion opportunities as a product, workflow, and value-based care expert.
Translate provider feedback into structured insights that inform product priorities and operational decisions.
Lead resolution of complex escalations by aligning internal teams around root causes, owners, and action plans.
Mentor Provider Success colleagues and contribute to the development of team standards, tools, and capabilities.

Providers can clearly articulate how Counterpart Assistant supports better patient care and success in value-based arrangements.
Your performance recommendations lead to observable changes in provider behavior, workflow execution, and relevant clinical or financial outcomes.
Your insights influence product enhancements, operating decisions, and provider-success strategy beyond your assigned portfolio.
Colleagues seek your guidance on complex provider situations, stakeholder management, and workflow transformation.
You Should Get in Touch If
You have 7–10+ years of experience in provider success, healthcare account management, clinical operations, practice transformation, value-based care consulting, or a related field.
You have managed complex or high-value
provider
relationships and can demonstrate success improving adoption, retention, satisfaction, and performance.
You have experience advising physicians, practice administrators, and healthcare leaders.
You have led significant changes to clinical or operational workflows in primary care or ambulatory care settings.
You can translate complex clinical, operational, and financial data into clear recommendations, performance plans, and executive-level narratives.
You understand how primary care organizations operate, including the competing demands placed on clinicians and practice staff.
You are comfortable leading through influence, resolving ambiguity, and coordinating stakeholders without direct authority.
You have strong program and project leadership skills and can manage multiple high-priority initiatives while maintaining accountability for outcomes.
You communicate effectively in provider meetings, performance reviews, product demonstrations, and challenging escalation conversations.
You are willing to travel up to 75% of the time to build relationships and support provider practices.
Preferred Qualifications
Deep knowledge of value-based care, Medicare Advantage, population health, or quality-improvement models.
Experience Implementing Healthcare Technology Within Primary Care Environments.
Familiarity with electronic health records, interoperability, medical billing, reimbursement, and risk-based payment arrangements.
Knowledge of annual wellness visits, risk adjustment and coding accuracy, Stars, HEDIS, and other quality-performance frameworks.
Experience designing provider-success playbooks, segmentation models, engagement programs, or performance-management processes.
Experience Mentoring Colleagues Or Serving As A Subject-matter Expert Within A Provider-facing Organization.
